docs(reflow): fact-check corrections and content fixes

- Remove inaccurate heading reconciliation example from how-it-works - Remove Text Structure Agent from sub-agent list (not a real sub-agent) - Fix extracted figures section: alt_text not yet populated, clarify fields - Remove confidence score from change ledger docs (not implemented) - Remove max_rounds from API docs (accepted but unused) - Fix estimated_completion_minutes to 5 (was 3) - Mark pipeline feedback endpoints as experimental/disabled in viewer - Update WordPress plugin install to use GitHub Releases - Add README index page for reflow docs Co-Authored-By: Claude Opus 4.6 <noreply@anthropic.com>
