docs: AGPL-3.0 license, contributing guide, and GitHub issue templates

- Add full GNU AGPL-3.0 LICENSE; set package.json license to AGPL-3.0-or-later; README license section explains the network-use copyleft (§13). - Add CONTRIBUTING.md and Contributor Covenant CODE_OF_CONDUCT.md. - Add .github issue templates: accessibility (a11y barriers in the app or in the produced HTML, with WCAG/AT/severity fields), bug report, feature request, a template chooser config (blank issues off, contact links), and a PR template. - Refresh README to describe the issue-based agent contribution flow (no PRs).
