chore(release): align version + docs for v0.1.0-beta.5 (#121)

- Bump version to 0.1.0b5 in pyproject.toml and src/main.py so the FastAPI /docs schema matches the v0.1.0-beta.5 tag. - Add PII Review as phase 1 in docs/reference/pipeline-phases.md (the viewer's PIPELINE_STAGES added it in #120; the reference doc needed to follow). - Add a "Releasing" section to AGENTS.md codifying the pre-tag checklist: bump version in both files, update pipeline-phases.md and AGENTS.md, merge first, tag second.
